ACP (Nurse/Paramedic) -The Junction Health Centre

Key Responsibilities:

• Assess, diagnose and manage patients autonomously in urgent and general practice settings, including ordering/tests and prescribing within competence.
• Provide holistic patient care and health promotion (e.g. chronic condition follow-up).
• Refer or discharge patients safely according to protocols and guidelines.
• Participate in clinic governance (clinical meetings, audits, case reviews).

• Completed Master’s degree in Advanced Practice.
